PLEASE NOTE

  • We are only able to accept referrals if you live within Te Matua a Māui (Hawkes Bay) region.

  • Hui are by appointment only.

  • Due to high demand, there may be a waiting period to begin receiving tautoko.

  • This referral may be delayed if this form is incomplete. Please note we do not accept referrals for tamariki under 4 years of age.

  • If we are unable to make contact after three attempts this referral will be closed and another referral will be required.
